Application Steps
STEP 1: The family will need a Veterinarian Referral in order for the Grant process to begin. This referral may ONLY be initiated by the veterinary practice (referral link can be access on Veterinarian’s Page). Families can, and should, request that their veterinarian fill out this referral.
** Veterinarian will need to determine if the pet falls under the medical conditions listed above and have an understanding of the pet’s owner commitment of their pet**
STEP 2: A Case Manager will contact the Veterinary Clinic by phone or email if additional information about the case is needed. It is important for Veterinarian to read For Veterinarian’s page.
** Requesting information from you does NOT automatically generate an “approval"“**
STEP 3: If the case meets the initial criteria for a grant, our Case Manager will send the client the unique password to fill out the grant application via email or text. Proof of income and picture of pet will need to be emailed to lexieslove1@gmail.com . Please note that we will not consider the grant application without having received the proof of income or similar as well as a picture of your pet. Click here to access Grant Application.
STEP 4: Once online application is submitted, and proof of income and the picture of pet is received, our Case Manager will then review all documentation against the Guidelines and refer the case to our Board for the final review. We will send an email to both the client and veterinary practice if case is approved or denied.
STEP 5: Once approved, the client will be eligible for a grant of up to $500 to be used specifically for the treatment cited on referral. Funds will be paid directly to the vet practice after we have received a copy of the invoice for services provided.
** Please be aware that we cannot prepay grant funds- funds are dispersed after services are performed, usually within 24 hours of receipt of the invoice **
